Most Used Forms

Inactivation/Termination of Course(s)The COURSE INACTIVATION OR TERMINATION REQUEST form is used to remove one or multiple courses from a department's list of active courses. "Inactivation" means a course could be reinstated; "termination" permanently removes it.Inactivation/Termination Request Form

Revising Existing CourseThe COURSE REVISION form is used for any and all changes to courses, whether the revisions are substantive or minor.Changes to Existing Course

Content Review CONTENT REVIEW form is needed for each pre-requisite, co-requisite, or advisory to the course being proposed or revised. UPDATE: As of September 2009, Content Review forms should be completed through the CATS curriculum database system. Use this form only for a draft. For further information, contact the Curriculum Office at 524-1554.

New Course Proposals

Permanent Courses, Transfer Level (1-99)The NEW PERMANENT COURSE PROPOSAL, Courses 1-99, is the form used for all new permanent UC and CSU transfer level courses.New 1-99 Course Proposal

Permanent Courses, Non-Transfer (100-399)The NEW PERMANENT COURSE PROPOSAL, Courses 100-399, is the form used for all new permanent non-transferable courses.New 100-399 Course Proposal

New Noncredit CoursesThe NEW PERMANENT NONCREDIT COURSE PROPOSAL, Courses 400-799, is the form used for all new permanent noncredit courses.

Changes to Existing Courses

Revising Existing CourseThe COURSE REVISION form is used for any and all changes to courses, whether the revisions are substantive or minor.Changes to Existing Course

Inactivating or Terminating Existing CoursesUse the CHANGES TO EXISTING COURSE form above and check the appropriate box. "Inactivation" means that the course will not be listed or scheduled, but it can be revised and reinstated later if it is needed. "Termination" means that the course may never be offered again under that number.

Supplementary Forms, as needed

Distance EducationDISTANCE EDUCATION form initiates a required review by the Distance Education Advisory Committee (DEAC). Any course that will be offered online or through other distance education means must submit this form. Please use the Distance Education Proposal Form through CATS, the curriculum database in the Student Information System.
